Today, we’re diving into a topic that might seem a bit confusing at first: how medications like Wegovy and Zepbound can increase insulin levels—the so-called ‘fat storage hormone’—yet still lead to significant weight loss. Sounds contradictory, right? Let’s break it down.

We’ve all heard that insulin is the ‘fat storage hormone’ and that carbs, which spike insulin, are the villains behind the obesity epidemic. This belief is rooted in the Carbohydrate-Insulin Model of Obesity, which claims:

  1. Eating carbs or sugar spikes insulin.
  2. Insulin prevents fat burning and stores fat and sugar in your fat cells.
  3. This leads to hunger, making you eat more and continuing the cycle.

The supposed solution? A low-carb or keto diet, promising a ‘Metabolic Advantage’ with increased metabolism and effortless fat loss. Sounds perfect, right? But there’s more to the story.

When you eat carbs, they turn into sugar in your bloodstream. Insulin then helps move this sugar into your cells, keeping your blood sugar levels in check. Yes, insulin also promotes fat storage—but only because it’s inefficient to break down fat when you have fresh nutrients from food.

Here’s the twist: insulin’s job isn’t to make you fat. It just helps manage energy efficiently. Plus, insulin isn’t the only hormone at work. Hormones like cortisol, epinephrine, and ghrelin balance insulin’s effects. Insulin levels rise mainly around meals and drop afterward, allowing other hormones to do their job.

Despite increasing insulin levels, Wegovy and Zepbound are effective for weight loss. Why? They help you maintain a calorie deficit by reducing appetite and food-seeking behaviors, making it easier to eat fewer calories.

So, insulin isn’t the villain, and carbs aren’t your enemy. You can enjoy carbs and still lose weight. The key is maintaining a calorie deficit with a diet you can stick to long-term. The relationship between insulin, your body, and food is complex and not as simple as carbs + insulin = weight gain.
